29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the exposing employer did not ensure that employees were utilizing appropriate eye protection while performing roofing operations with pneumatic nail guns.  This condition exposed employees to eye injuries including the introduction of foreign particles into the eye while working.

29 CFR 1926.501(b)(11): Employees conducting roofing operations on a steep roof with unprotected sides and edges 6 feet or more above lower levels, were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   a)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the exposing employer did not ensure employees were protected from falls by an adequate fall protection system while performing roofing operations on a steep pitch (5/12) commercial property. This condition exposed employees to fall hazards greater than six feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the employer did not ensure that the 16 foot Werner, type II,  portable extension ladder located on the southeast side of the building, being used for access to the roof, was extended at least three feet above the upper landing surface of a commercial roof. This condition exposed employees to fall hazards associated with ladder access.  b)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the employer did not ensure that the 20 foot Werner, type II,  portable extension ladder located on the south side of the building, being used for access to the roof, was extended at least three feet above the upper landing surface of a commercial roof. This condition exposed employees to fall hazards associated with ladder access.  c)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the employer did not ensure that the Werner portable ladder located on the northeast side of the building, being used for access to the roof, was extended at least three feet above the upper landing surface of a commercial roof. This condition exposed employees to fall hazards associated with ladder access.
