This condition exposed employees to fall hazards associated with ladder access. b)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the employer did not ensure that the 20 foot Werner, type II, portable extension ladder located on the south side of the building, being used for access to the roof, was extended at least three feet above the upper landing surface of a commercial roof. This condition exposed employees to fall hazards associated with ladder access. c)Lions Rock Roofing, LLC at 204 East Holly Boulevard, Brandon, SD 57005: On or about and at times prior to May 10, 2024, the employer did not ensure that the Werner portable ladder located on the northeast side of the building, being used for access to the roof, was extended at least three feet above the upper landing surface of a commercial roof.
