29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) On or about May 9, 2024, on the west side of the jobsite, employees utilized an SPX Hydraulic Power Unit while conducting concrete work on the foundation of the residential property that was plugged into an electrical receptacle with reverse polarity.
