29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  a- 146 Norwich Street, Woolwich Township NJ: Company workers were exposed to struck by hazards when using  pneumatic nailers without eye or face protection while performing roof shingle installation and other related activities at the worksite. Condition observed on or about 05/13/24.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a- 146 Norwich Street, Woolwich Township NJ:  The company worksite Foreman and their workers were exposed to fall hazards, in excess of 10 feet 4 inches to ground surface, when accessing to and egressing from a residential structure's pitched porch roof while using a portable, unsecured, aluminum extension ladder whose side rails extended approximately 13 inches above the surface being accessed to and egressed from.  Condition noted on or about 05/13/24.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a- 146 Norwich Street, Woolwich Township NJ:  A worker and the company owner were exposed to fall hazards, in excess of 10 feet 2 inches and 12 foot 7 inches to the ground's surface below, when walking and working from a residential structure's pitched porch roof while either wearing a full body harness that was not connected to an anchor with a lanyard or when not using fall protection at all.   Condition observed on or about 05/13/24.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
