29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  a)	On or about May 14, 2024, the employer failed to ensure the energy control procedure for the Hovis A Punch Press Machine clearly and specifically outlined the intended use of the energy control procedure for the machine [per 1910.147(c)(4)(ii)(A)], included specific procedural steps for shutting down, isolating, and securing the electrical and pneumatic energy sources to the Hovis A Press [per 1910.147(c)(4)(ii)(B)], included procedural steps for the placement and removal of lockout devices for the electrical and pneumatic power energy isolation devices at the Hovis A Press, [per 1910.147(c)(4)(ii)(C)], and included specific requirements for testing the press to determine and verify the effectiveness of the lockout devices and energy control measures [per 1910.147(c)(4)(ii)(D)], for employees engaged in service and maintenance activities, such as but not limited to, setting up operations.  The employer's procedure was not specific to the Hovis A Punch Press Machine and only included how the machine's energy isolation sources were identified and labelled.     b)	On or about May 14, 2024, the employer failed to ensure the energy control procedure for the Rousselle Mechanical Power Press, known as Press #9, clearly and specifically outlined the intended use of the energy control procedure for the machine [per 1910.147(c)(4)(ii)(A)], included specific procedural steps for shutting down, isolating, and securing the electrical and pneumatic energy sources to Press # 9 [per 1910.147(c)(4)(ii)(B)], included specific procedural steps for the placement and removal of lockout devices for the electrical and pneumatic power energy isolation devices at Press #9  [per 1910.147(c)(4)(ii)(C)], and included specific requirements for testing the press to determine and verify the effectiveness of the lockout devices and energy control measures [per 1910.147(c)(4)(ii)(D)], for employees engaged in service and maintenance activities, such as but not limited to, setting up operations. 29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about May 14, 2024, the employer failed to ensure periodic inspections of their energy control (lockout/tagout) procedures were conducted at least annually for the Hovis A Punch Press Machine, Rouselle Mechanical Power Press, known as Press #9, and other machines and equipment at the facility, to ensure the procedures and the requirements for hazardous energy control (lockout/tagout) were being followed, to ensure any deviations or inadequacies of the procedures were identified [per 1910.147(c)(6)(i)(B)], a review of authorized employee's responsibilities under the energy control procedures [per 1910.147(c)(6)(i)(C)], and a certification the periodic inspections were completed [per 1910.147(c)(6)(ii)].

29 CFR  1910.217(b)(7)(iii):A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) shall be supplied with the clutch/brake control to select type of operation of the press. Fixing of selection shall be by means capable of supervision by the employer.  On or about May 14, 2024, the employer failed to ensure the means for selecting Off, Inch, Single, and Continuous mode of operation on the Rousselle Mechanical Power Press, known as Press #9, was capable of supervision by the employer.  The key used to select the various modes of operation was left in the selector switch and capable of being changed by the operator and/or other employees.

29 CFR  1910.217(b)(7)(ix):If foot control is provided, the selection method between hand and foot control shall be separate from the stroking selector and shall be designed so that the selection may be supervised by the employer.  On or about May 14, 2024, the employer failed to ensure the selection method between hand and foot control on the Rousselle Mechanical Power Press, known as Press #9, was capable of supervision by the employer.  The key used to select between hand and foot control (actuation) was left in the selector switch and capable of being changed by the operator and/or other employees.
