29 CFR  1910.269(c)(3)(ii): Additional job briefings were not held when significant changes occurred during the course of the work, affecting the safety of the employees, .  NAWS inside CL airfield-controlled area: Thompson Laboratory:  On May 8, 2024, employees were replacing both the lower 34.5kV and 4.8kV cross arms on pole NS-63 when a ground high voltage electrician saw that the center jump wire was not attached which created a significant work change. The employer did not ensure the employees held an additional brief to address the condition which potentially exposed employees to arc flash burns.

29 CFR  1910.269(l)(8)(v): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective clothing and other protective equipment that had an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 Cal/cm2.   NAWS inside CL airfield-controlled area: Thompson Laboratory:  On May 8, 2024, employees conducted high voltage electrical repair operations replacing both the lower 34.5kV and 4.8kV cross arms on pole NS-63 without wearing the appropriate personal protective equipment (PPE). The employer did not ensure that employees wore required personal protective equipment.  This condition potentially exposed employees to arc flash burns.

29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  NAWS inside CL airfield-controlled area: Thompson Laboratory:  On May 8, 2024, employees conducted high voltage electrical repair operations replacing a cross arm on pole NS-63 without job hazard assessments conducted for all tasks. The employer did not ensure a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done. This condition potentially exposed employees to  arc flash burns.
