29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Homag single edge bander, backside with blue colored hoods; On or about May 8, 2024, and dates prior, the written energy isolation procedures for controlling electrical, pneumatic, and thermal energy were not utilized while employees performed servicing activities of cleaning tailings/debris out and setup activities of changing/adjusting cutters inside the edge bander enclosure. Rather, operators used an unwritten procedure that involved them turning a control key to the off position and waiting to open enclosure access hoods until the door/hood interlocks timed out before they entered the Homag single edge bander to perform servicing actives of cleaning tailings/debris out and changing/adjusting cutter heads inside the machine enclosure. (b) Homag single edge bander, product feed/conveyor side; On or about May 8, 2024, an operator did not utilize the energy isolation procedure to control electrical and pneumatic energy before cleaning debris from the back/conveyor side of the Homag edge bander with an air wand and their hand. The operator received a partial finger amputation injury when their fingers contacted an energized cutter head, as they reached inside the machine.

On or about May 14, 2024, the employer did not provide the required description in column F of their calendar year 2024 OSHA Form 300 to identify the object or substance that directly caused injury and/or the extent of injury. Additionally, injury classification information was not completed as required by recording/marking the days away from work (column K) and/or on job transfer/restriction (column L). (a) On or about April 17, 2024, an employee working at 88-Assembly Line 1 and 2 strained their left wrist, case no. IN-20240417-002. The object or substance that directly injured the person was not described and the number of days transferred or restricted was not identified. (b) On or about May 13, 2024, an employee in the north breakroom received a head injury from a projector screen that tipped over in the closet, case no. IN-20240513-001. The extent of injury was not described and the number of days away from work was not identified.
