29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May 15, 2024, at a residential construction site, employees were exposed to fall heights greater than 6 feet while performing roofing work on the roof of a two-story house without the use a fall protection system.

29 CFR 1926.1051(a):Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On or about May 15, 2024, at the residential construction site, employees were trying to get on and off the upper part of the residential home being constructed without proper access.
