29 CFR 1910.23(c)(10): The employer did not ensure that the top of a non-self-supporting ladder is placed so that both side rails are supported, unless the ladder is equipped with a single support attachment. On or about May 7, 2024, at a service tap located in the alley behind 401 N 12th St, Muskogee, OK, the strand being used to support a portable ladder came loose and did not adequately support the ladder when an employee was using the ladder.
