29 CFR 1904.32(b)(3): A company executive did not certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete: USDA Forest Service (Jemez Ranger District), 051 Woodsy Lane Jemez Springs, NM 87025: On or about May 15, 2024, for calendar year 2021, 2022, and 2023 the employer did not certify and sign the OSHA Form 300A Summary of Work -Related Injuries and Illnesses by a company executive. The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.32(b)(3), which was contained in OSHA inspection number 1605573 citation number 1, item number 1 and was affirmed as a final order on August 05, 2022, with respect to a workplace located at 4 Lost Lodge Road, Cloudcroft, NM 88317.
