29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: (a) Marshfield Medical Center - Hospital main floor; On or about May 8, 2024, an employee was using the top step (third rung) of a 4-foot Westward (WW-3010-04) stepladder when the ladder became unstable and tipped, causing the employee to fall to the floor.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder: (a) Marshfield Medical Center - Hospital main floor; On or about May 8, 2024, an employee was descending from the top step (third rung) of a 4-foot Westward (WW-3010-04) stepladder when the ladder became unstable and tipped, causing the employee to fall to the floor. While descending, the employee did not maintain at least one hand on the ladder to maintain stability.
