Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse.   a) Ammo International, Inc.- Warehouse/storage areas:  The employer did not ensure that the product was secured properly to prevent against sliding and collapse.  An incident occurred where employees suffered crushing hazards from collapsed boxes falling on top of them, the boxes contained product that weighted approximately 500 lbs. in total.  Incident occurred on 5/7/ 2024.  b) Ammo International, Inc.- Warehouse/storage areas: Employees were exposed crushing injuries and falls from heights while climbing onto handmade wooden racks and structures to retrieve boxes of products that do not have a weight rating and are not secured properly, occurred on or about 5/15/2024.
