The employee was exposed to falls of up to 13 feet to the concrete slab.

Employees were exposed to falls of up to 22 feet.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) 117 NY 14 A (Millard Street), Dundee, NY  14837, On or about 6/5/24:  Workers were installing a metal roof system, on a 3.75:12 pitch roof,  without fall protection of any kind.  Employees were exposed to falls of 18 feet to ground.
