29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about May 15, 2024, employees operating pneumatic nail guns while conducting framing work were not protected from eye hazards such as flying particles or nails.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 15, 2024, portable ladders used to access upper surfaces of the partially framed house did not extend at least 3 feet (0.9 m) above the working surfaces which the ladders were used to gain access to.
