29 CFR 1910.30(a)(3): The employer did not provide employees training in at least the following topics; the procedures to minimize fall hazards; the correct procedures for installing, inspecting, operating, maintaining, and disassembling personal fall protection systems that the employee uses; and the correct use of personal fall protection systems and equipment. Employer did not adequately train employees on fall protection and the appropriate methods for inspecting PFAS lanyards. Employees were exposed to fall hazards greater than 11 feet.

29 CFR 1915.73(d): When employees are exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges shall be guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2), unless the nature of the work in progress or the physical conditions prohibit the use or installation of such guardrails. Employer removed a guardrail from the boat four-deck section, exposing employees to 11 feet fall hazards
