The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: At the workplace located at 220 Kindleberger Road, Kansas City, Kansas 66115, the annual periodic review of the lockout / tagout program was not completed to identify and address deficiencies within the written program or during an evaluation of employees performing maintenance or preventive maintenance on hard wired equipment, thus exposing employees to struck-by, crushing, and electrical hazards.

29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: At the workplace located at 220 Kindleberger Road, Kansas City, Kansas 66115, employees were exposed to amputation hazards. The employer did not ensure that employees who were testing the operation of the silo 2 rotary valve, de-energized and applied a lockout device to the equipment prior to continuing the servicing and maintenance task.
