29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about May 15, 2024, at the above addressed jobsite, an employee performing residential roofing operations, nineteen (19) feet eight (8) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a.  On or about May 15, 2024, at the above addressed jobsite, an employee wearing a personal fall arrest system while performing residential roofing operations was not attached to the lanyard and rope by the D-ring in the center of the employee's back near shoulder level, thereby exposing the employee to fall hazards.
