29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: On or about May 15, 2024, at 8679 South Freeway Drive, Macedonia, OH 44056, the employer failed to ensure that employees operating tanker trucks were protected from explosion/chemical burn hazards in that employees were not trained on the physical hazards associated with corrosive liquid chemicals including but not limited to Cerfa-Kleen L-561 such as the ability to react with aluminum to generate hydrogen gas.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: On or about May 15, 2024, at 8679 South Freeway Drive, Macedonia, OH 44056, the employer failed to ensure that employees operating tanker trucks were trained on measures that they can take to protect themselves from chemical hazards, including prohibiting the loading of corrosive liquid chemicals including but not limited to Cerfa-Kleen L-561 into unapproved containers..
