29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were maintained in a legible condition:  Employees were exposed to struck-by hazards.  Employees engaged in material handing activities using forklifts were exposed to stuck-by hazards in that the following markings were not legible: a)	Hyster forklift, serial number C010H02098S, all 3 lever indicator labels were faded, eligible to read. b)	Hyster forklift model number S50XM, serial number D187V19347X, all 3 lever indicator labels were not legibly marked.

1910.303(b)(2): Electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Employees performing CNC machining operations in the middle of the main building at the machining area were exposed to electrical shock, and fire hazards in that the Relocatable Power Taps (power strips) were series-connected (daisy-chained) to other power strip to energize the battery chargers.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  Employees performing cleaning parts activities in the Tumbler Room were exposed to electrical shock, and fire hazards in that the outlet next to the door leading to the other room, by the T1 machine had a missing faceplate.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure:  Employees performing CNC machining operations in the middle of the main building at the machining area, and the Tumbler Room were exposed to electrical shock, and fire hazards in that the following appliances at the following locations were plugged into the extension cords in lieu of permanent wiring. They were using as a substitute for the fixed wiring of a structure. a)	A flexible cord running from the dropdown outlet to the back of the Lathe machine L4 was utilized to energize the light for the machine. b)	A flexible cord running from above outlet to the back of the Haas CNC machine H17 was utilized to energize the battery charger. b)	A flexible cord running from the missing faceplate outlet to the mobile rack inside the Tumbler Room was utilized to energize the fan.
