29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about May 13, 2024, on the roof of a commercial structure located at 4445 Pine Forest Drive, Lake Worth, Florida, an employee performing air conditioning unit replacement work was exposed to a fall hazard of 19 feet-7 inches when working adjacent to a skylight while not being protected from falling to the lower level by personal fall arrest systems, covers, or guardrail systems erected around the skylight.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about May 13, 2024, on the roof of a commercial structure located at 4445 Pine Forest Drive, Lake Worth, Florida, the employer's competent person failed to identify an anticipated and existing 19 feet-7 inches fall hazard, in that skylights on the roof of the structure in the vicinity of the work taking place were not covered or had barricades placed around them to protect employees from falling through those skylights.
