29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:   a.	On or about May 16, 2024, employees were using a safety work platform to access the roof, the employees were exposed to fall and crush-by hazards in that the platform was not attached to the JLG lift.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a.	On or about May 16, 2024, the employer exposed employees to a 26-foot fall hazard in that they were not wearing safety harness and lanyards attached to the safety platform that was attached to a rough terrain forklift and used to raise employees up to the roof.

On or about May 16, 2024, the employer exposed employees to a 26-foot fall hazard in that the safety work platform attched to a JLG rough terrain forklift and used to access the roof was missing a guard rail on the end of the platform.

29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.   a. On or about May 16, 2024, on a 2:12 pitch roof, the employer exposed two employees performing commercial roofing operations to a 26-foot fall hazard from the roof edges in that they were not protected by a fall protection system.
