29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a. On or about May 16, 2024, employees were using a safety work platform to access the roof, the employees were exposed to fall and crush-by hazards in that the platform was not attached to the JLG lift.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. On or about May 16, 2024, the employer exposed employees to a 26-foot fall hazard in that they were not wearing safety harness and lanyards attached to the safety platform that was attached to a rough terrain forklift and used to raise employees up to the roof.
