29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. On or about May 15, 2024, employees were performing roofing operations on the northeast roof and north roof and were standing on a roof bracket scaffold approximately 13.9-22.9 feet above ground. The employees were not using any means of fall protection. b. On or about May 15, 2024, employees utilized a walkthrough scaffold for access to the the roof bracket scaffold and were exposed to a fall hazard of approximately 22.9 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 15, 2024, at the above addressed jobsite, the portable ladder used to access the scaffold did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to fall hazards of approximately 22.9 feet. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
