GrafTech International Holdings Inc., Pitch Tank 001 in the Mill, Mix, and Forming (MMF) area, on or about May 17, 2024 and times prior thereto: The danger sign on Pitch Tank 001 in the MMF area included red lettering painted on a rusty surface. The colors red, black, and white were not utilized in combination to make the sign.

GrafTech International Holdings Inc., Green Scrap Crusher Elevator in the Mill, Mix, and Forming (MMF) area, on or about May 17, 2024 and times prior thereto: The Green Scrap Crusher Elevator in the MMF area did not contain a legible sign to inform employees of the existence, location of, and danger posed by the permit confined space.
