29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Innovive, LLC at 301 Centennial Drive #100, North Sioux City, SD 57049: On or about and at times prior to May 15, 2024, the employer did not ensure the front of the conveyor system at the Line 1 denester was guarded. This condition exposed employees to caught-in and amputation hazards associated with the unguarded end of the conveyor. (b)Innovive, LLC at 301 Centennial Drive #100, North Sioux City, SD 57049: On or about and at times prior to May 15, 2024, the employer did not ensure the front of the conveyor system on the Line 2 denester was guarded. This condition exposed employees to caught-in and amputation hazards associated with the unguarded end of the conveyor. (c)Innovive, LLC at 301 Centennial Drive #100, North Sioux City, SD 57049: On or about and at times prior to May 15, 2024, the employer did not ensure that an adequate guarding system was in place on the Line 2 denester. This condition exposed employees to caught-in and amputation hazards associated with the unguarded machinery. (d)Innovive, LLC at 301 Centennial Drive #100, North Sioux City, SD 57049: On or about and at times prior to May 15, 2024, the employer did not ensure the Line 1 denester was adequately guarded on the right side of the interlocked door area. This condition exposed employees to caught-in and amputation hazards associated with inadequate guarding.

29 CFR 1910.219(h)(1): Projecting keys, setscrews, or other projections in revolving parts were not removed, made flush, or guarded by metal covers: (a)Innovive, LLC at 301 Centennial Drive #100, North Sioux City, SD 57049: On or about and at times prior to May 15, 2024, the employer did not ensure the spinning shaft with projecting set screws on the Line 1 Denester conveyor belt system were properly guarded. This condition exposed employees to caught-in hazards associated with the unguarded set screws. (b)Innovive, LLC at 301 Centennial Drive #100, North Sioux City, SD 57049: On or about and at times prior to May 15, 2024, the employer did not ensure the spinning shaft with four projecting set screws on the Line 2 Denester conveyor belt system were properly guarded. This condition exposed employees to caught-in hazards associated with the unguarded set screws.
