The forklift was not taken out of service, exposing employees to struck-by hazard. (b) On or about 05/15/2024, at Crown Bay, Lot 170-3, St Thomas, VI 00803 - Crowley Yard, a Toyota Forklift Truck Model 8FD45U sit-down power industrial truck had damaged tires. The forklift was not taken out of service, exposing employees to struck-by hazards.
