29 CFR 1910.37(e): Employer did not install and or maintain an operable employee alarm system that had a distinctive signal to warn employees of fire or other emergencies: (a) Mount Ida Hydro Station, Control room platform: The employer did not maintain the employee alarm system in operating condition in the event of an emergency in that the fire alarm system in the hydro dam had indicated faults and warning lights.

29 CFR 1910.157(e)(1): The employer did not do inspection, maintenance and testing of all portable fire extinguishers in the workplace: (a) Mount Ida Hydro Station, Control room platform: On or about May 21, 2024, the employer did not maintain and test the Badger Model # B10HA CO2 fire extinguisher located on the control platform near the incident scene. The Badger Model # B10HA CO2 fire extinguisher service tag showed last service in 2008.

29 CFR 1910.269(a)(3)(i)(A): The host employer did not ensure that each of its employees were instructed in the hazardous conditions relevant to the employee's work that the contract employer was aware of as a result of information communicated to the contract employer by the host employer under paragraph (a)(3)(i) of this section: (a) Mount Ida Hydro Station, Control room level: On or about May 21, 2024, sub-contractor employees conducting maintenance on an Allis Chalmers MA-250C circuit breaker in a 4160-volt switchgear housing experienced an arc flash when the breaker contacted an energized cell during racking-in. The host employer did not inform the subcontractor of the characteristics of the host employers installation, including but limited to the nominal voltages of lines and equipment and maximum switching transient voltages.
