29 CFR 1910.147(e)(2)(i):The work area shall be checked to ensure that all employees have been safely positioned or removed. (a) R73 rewinder - On or about May 16, 2024, employees working inside the R73 rewinder performing trouble shooting, when the supervisor instructed the operator to restart the machine but failed to check the machine to see if any employees were still in the machine before restarting exposing employees to caught in hazards.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: (a) R73 rewinder - On or about May 16, 2024, the employer failed to use the lockout procedure that requires authorized employees to place locks on all energy source while performing the troubleshooting procedure on the R73 exposing employees to caught in hazards.
