29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) 3741 Montana Ave, Billings MT: On or about May 10, 2024, and at times prior, employees were exposed to struck by hazards while operating a Clark GCS25MC Forklift that had been modified by drilling holes in the forks so that trailers could be attached and moved throughout the worksite.
