29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a. On the NS1 transfer station platform and conveyor catwalk located at 1915 Wigmore Street, Jacksonville, Florida: On or about May 13, 2024, the employer exposed night shift supervisor and 2-other employees to fall hazards, in that the walking working surface with an unprotected side that is 4 feet or more above a lower level did not have a complete guardrail system and no other fall protection systems had been utilized.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. At the NS1 transfer point drive motor for the head end of the NS1 transfer conveyor, located at 1915 Wigmore Street, Jacksonville, Florida: On or about May 13, 2024, the employer exposed employees to amputation hazards, in that the conveyor drive motor had not been locked and tagged out by the authorized users while replacing conveyor drive motor belts. b. At the jetty chute conveyor drive motor, located at 1915 Wigmore Street, Jacksonville, Florida: On or about May 13, 2024, the employer exposed employees to struck-by material hazards, in that the conveyor drive motor had not been locked and tagged out by the authorized users while cleaning out the jetty chute below the jetty conveyor.
