The employee(s) were conducting maintenance activities while fuel-gas cylinders were permanently stored together with oxygen cylinders in the compressed gas storage area of the facility.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets:  Employee(s) are exposed to chemicals that can cause eye and skin irritation. This was most recently documented on or about May 21, 2024, at 42840 U.S. 20, Ainsworth, Nebraska. Employee(s) are utilizing chemicals including but not limited to Aerosol paints, Brakleen, Anti-seize lubricant, Propane and Acetylene related to trailer maintenance and repair activities.   Instances of inadequate training included but were not limited to:  i.     Maintenance employees using bearing lubricant for packing bearings. ii.    Maintenance employees using acetylene for welding. iii.   Maintenance employees using brake cleaner and anti-seize for trailer maintenance.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  The employer did not report to OSHA within 24 hours an employee in-patient hospitalization. The in-patient hospitalization occurred on April 24,2024. This violative condition was documented on May 21, 2024, at or about 42840 U.S. 20, Ainsworth, Nebraska.
