29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a)	Virginia International Gateway Terminal: On or about May 21, 2024, the NAPA 220A engine starter and battery changer was not visually inspected prior to use in that the charger was plugged into a wall mounted receptacle with a missing ground prong, thereby exposing employees to a possible electrical shock hazard.  b)	Marine Repair Service near terminal: On or about May 21, 2024, the flexible cord to the pedestal fan to was not visually inspected prior to use in that the male attachment plug was missing its ground prong, thereby exposing employees to a possible electrical shock hazard.  No abatement certification or documentation required.

29 CFR 1917.43(e)(6): Employees were elevated by fork lift trucks without using a platform secured to the lifting carriage or forks:   a)	Virginia International Gateway Terminal: On or about May 21, 2024, an employee was observed servicing a refrigeration unit standing in a basket approximately 5ft from the ground that was not properly secured to the Caterpillar model 2PD6000 forklift, thereby exposing employee to a possible fall hazard.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1917.43(e)(6)(i): Platforms used on fork lift trucks to elevate employees did not have railings complying with 29 CFR 1917.112(c):  a)	Virginia International Gateway Terminal: On or about May 21, 2024, an employee was observed in an elevated basket attached to a Caterpillar model 2PD6000 forklift with a broken top chain rail while servicing a refrigeration unit attached to a shipping container, thus exposing the employee to a possible 5ft fall hazard.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)   Virginia International Gateway Terminal: On or about May 21, 2024, a relocatable power tap was not used in accordance with its listing and labeling in that the male attachment plug to the powder tap was plugged into the female end of an extension cord to supply power to a portable electric drill, battery chargers, and other low voltage equipment.    b)   Marine Repair Services: On or about May 21, 2024, a galvanized steel, 4 x 4 square junction box with a single receptacle hardwired to an extension cord used to supply 440 volts to a plasma machine was not used in accordance with its listing and labeling in that the box was not mounted.  No abatement certification or documentation required.
