29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On a residential roof, at a temporary jobsite located at 4052 Heath Road, in Jacksonville, FL: On or about May 21, 2024, the employer exposed subcontracted employees to fall hazards, in that fall protection was not utilized by subcontracted employees while performing roofing work.

29 CFR  1904.41(a)(1)(i): The establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date:   a. Total Home Roofing, LLC., located at 597 Haverty Court, Rockledge, FL 32955: On or about May 21, 2024, the employer failed to electronically submit information from their OSHA Form 300A, Summary of Work-Related Injuries and Illnesses or equivalent, for calendar year 2023 on or before March 2, 2024. The establishment employed approximately 132 employees and was classified in the North American Industrial Classification System as 238160.
