29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 37 Lightning Bug Lane, Paw Paw, West Virginia: On or about May 14, 2024, an employee received injuries that resulted in an amputation after the employee's hand was caught between a dumpster and a locking bar. The amputation was not reported to OSHA within 24 hours.

29 CFR 1910.151(b):There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: a) 37 Lightning Bug Lane, Paw Paw, West Virginia: On or about May 14, 2024, employees operating refuse collection vehicles utilizing compaction equipment, winch systems and locking levers were without medical services in near proximity and were not provided with or have an employee who is currently trained in first aid on the vehicle.
