On or about May 22, 2024, at location 2335 Union Avenue, Sheboygan, WI 54180: In the warehouse, pre-kit department, a one Gang box was not installed and used in accordance with the instructions outlined in the listing or labeling for the metal box. The metal box was installed as a pendant, attached to a flexible cord, rather than mounted to, or within a wall as required by the listing and labeling.

On or about May 22, 2024, at location 2335 Union Avenue, Sheboygan, WI 54180: In the warehouse, pre-kit department, a one Gang box installed as a pendant, had strain on the cable that pulled directly at both ends of the terminal joints exposing the hot, ground, and equipment grounding conductors within the cable.
