OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards from using a Ship's Ladder at a greater than 70 degree slope from horizontal: a) Worksite - On or about May 21, 2024, employees climbed a Ship's Ladder at the site to access roof to do roofing work with the Ship's Ladder being installed at over a 75 degree angle from horizontal (closer to 76 degrees) approximately exposing employees to fall hazards up to approximately 14 feet 2.6875 inches in height.

29 CFR 1926.1052(c)(9): Handrails for stairways did not provide adequate handholds for employees grasping them to avoid falling: a) Worksite - On or about May 21, 2024, employees climbed a Ship's Ladder at the site to access roof to do roofing work with the Ship's Ladder being installed with handrails that were right up against the hatchway at the top of the ladder that did not provide employees with adequate handholds for employees to grasp exposing employees to fall hazards up to approximately 14 feet 2.6875 inches in height.
