The CDZ was not marked by the use of control lines or the equivalent. On or about May 17, 2024, at the eastside of the jobsite, at a building roof, employees were on the roof for metal decking work and the boundaries of the controlled decking zone were not clearly marked.

On or about May 17, 2024, at the eastside of the jobsite, at a building roof, an employee worked at a controlled decking zone and was not provided training on controlled decking zones.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about May 17, 2024, at the east side of the jobsite, an employee fell from a roof, was hospitalized as an in-patient within 24-hours of the fall, and the employee hospitalization was not reported to the local OSHA area office within the 24- hour reporting requirement.
