29 CFR  1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Oshikiri Corporation of America Warehouse - 10425 Drummond Rd. Philadelphia, PA 19154 - On or about May 22, 2024, the employer failed to ensure that the machine guards adequately protected employees from having any part of their body in the danger zone during operating cycles on the two Radial Drilling Machines, exposing employees to puncture and amputation hazards.  b) Oshikiri Corporation of America Warehouse - 10425 Drummond Rd. Philadelphia, PA 19154 - On or about May 22, 2024, the employer failed to ensure that the machine guards adequately protected employees from having any part of their body in the danger zone during operating cycles on the three drill presses, exposing employees to puncture and amputation hazards.  c) Oshikiri Corporation of America Warehouse - 10425 Drummond Rd. Philadelphia, PA 19154 - On or about May 22, 2024, the employer failed to ensure that the machine guards adequately protected employees from having any part of their body in the danger zone during operating cycles on the horizontal bandsaw, exposing employees to amputation hazards.  d) Oshikiri Corporation of America Warehouse - 10425 Drummond Rd. Philadelphia, PA 19154 - On or about May 22, 2024, the employer failed to ensure that the machine guards adequately protected employees from having any part of their body in the danger zone during operating cycles on the bender machines, exposing employees to amputation hazards.

29 CFR  1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  a) Oshikiri Corporation of America Warehouse - 10425 Drummond Rd. Philadelphia, PA 19154 - On or about May 22, 2024, the employer failed to ensure that the compressed air used for cleaning machinery and equipment was reduced to 30 p.s.i. or less.  Employees used compressed air measured at 70 - 100 p.s.i. to clean off products and equipment continuously throughout the day, exposing employees to flying debris hazards.  b) Oshikiri Corporation of America Warehouse - 10425 Drummond Rd. Philadelphia, PA 19154 - On or about May 22, 2024, the employer failed to ensure that employees were not cleaning themselves off with compressed air from an air hose.  Employees cleaned themselves off with the compressed air on a daily basis before the end of their shift, exposing employees to flying object hazards.
