29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  1014 Southampton Road, Westfield, MA: On May 23, 2024, and at times prior, the employer had not established an energy control program to protect employees who performed maintenance and servicing activities on building machinery.  *Abatement Notes:  Energy control procedures are required to be documented for all multiple-energy source machines, such as but not limited to, the Matsuura MC-800V-DC Twin Spindle.   Energy control training is required to be documented and provided for all employees who are, or may be, in an area where energy control procedures may be utilized. A periodic inspection shall include the following elements:  - Appointment of an authorized employee to inspect each local procedure for inaccuracies during an actual energy control task - A review by the inspector with each authorized employee, of that employee's responsibilities under each energy control procedure they are authorized to use - Certification that periodic inspections have been performed by identifying the machines or equipment on which the energy control procedures were being utilized, the dates of the inspections, the employees included in the inspections, and the persons performing the inspections *Abatement notes are intended only to provide clarity regarding existing requirements under the law or agency policies.  Abatement notes do not create (or diminish) legal obligations under the Occupational Safety and Health Act

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  1014 Southampton Road, Westfield, MA: On May 23, 2024, and times prior, employees operated machines that were not provided with guarding that protected employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a.  Employees operated older model CNC machines such as but not limited to the Matsuura MC760V CNC 25 tool Machining center that did not have interlocks in place to turn off cutting tool when doors of the CNC machines were opened.  b.  Employees were exposed to unguarded nip points of a rotating LNS Turbo Microfine chip conveyor located on the North side of the machine floor.  c.  Employees were exposed to an unguarded rotating shaft on an SL-2 Mori Seiki CNC Lathe located on the North side of the machine floor.
