29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Worksite, 843 Lake Blvd, Lindenhurst, NY, 11757 - Two employees on site were installing roof rafters on a multi-story multi-residential structure. Two employees were standing on the same plank of a job made scaffold that was not fully planked approximately 10-feet above the lower level on the interior of the structure; on or about May 23, 2024.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The employees were utilizing a job made scaffold and standing on a single plank approximately 10-feet above the lower level. The plank was not at least 18 inches wide; on or about May 23, 2024.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

Employees were not utilizing a means of fall protection while working from the top plate of the exterior wall and were  exposed to falling approximately 20-feet to the ground below; on or about May 23, 2024. 29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) Worksite, 843 Lake Blvd, Lindenhurst, NY, 11757 - Employees were using a Werner 8-foot a frame ladder to gain access to the 2nd floor exterior top plate of a 2-story multi-residential dwelling. The ladder was used in the closed position,  leaning against the wall and employees were stepping on the top cap of the ladder to access the work area; on or about May 23, 2024.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19
