29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a.  On or about May 23, 2024, at the above addressed jobsite, employees performing water main installation activities in a trench, five (5) feet eleven (11) inches in depth, were not utilizing cave-in protection, thereby exposing the employees to cave-in hazards.
