The competent person was not competent in that he did not know a warning line system could only be used in conjunction with a personal fall arrest system, guardrail system, safety net system or a safety monitoring system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50-feet (15.25 m) or less in width, the use of a safety monitoring system alone: a) On or about May 22, 2024, on the flat roof of St. Bernadette Catholic Church, employees were performing thermoplastic polyolefin membrane installation work at heights greater than 6 feet without adequate fall protection. A warning line system was used without a personal fall arrest system, guardrail system, safety net system or a safety monitoring system.
