At the construction site, the employer did not ensure that employees were wearing hardhats while working below other employees. Employees were exposed to a struck-by hazard from falling objects.

At the jobsite, employees operating a Pneutools nail gun to perform framing activities without eye and face protection exposing him to the hazard of being struck-by flying objects.

29 CFR  1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.  At  the construction site, the employer did not provide an extension ladder for employees to access and egress the upper levels of the residential structure, exposing  employees to a fall hazard.

The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
