29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about May 23, 2024, the employer failed to ensure an energy control procedure was developed and documented for the #3 WEINIG Profimat 23 Moulder. A production operator was exposed to unexpected energization of hazardous energy from rotating parts, while performing maintenance and or servicing activities such as clearing jams and blade changes.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate and control. a. On or about May 23, 2024, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Machine operators were exposed to amputations from rotating spindles with blades and moving parts from rollers while cleaning, unjamming, or performing blade changes at the #3 Weinig Profimat 23 Moulder.
