29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative. a) Delicious Orchards and Orchard Markets Inc.- Bakery area: The employer did not establish a Lock Out/Tag Out Program consisting of energy control procedures, employees training, and periodic inspections to protect employees against the unexpected energization of equipment when servicing, making adjustments, replacing parts, cleaning or unjamming machines, including but not limited to, LVO sheeter/moulder model # SM20-4, S/N122-1030KB1, occurred on or about 05/17/2024.
