29 CFR 1910.253(a)(4): Workmen in charge of the oxygen or fuel gas supply equipment, including generators, and oxygen or fuel gas distribution piping systems were not instructed and judged competent by their employers for this important work before being left in charge: a) On or about May 18, 2024, in the bed of the pickup truck, where employees adjusted the regulator on the acetylene tank without first being properly trained.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) On or about May 18, 2024, at approximately 7:00 p.m., an employee received burns at the jobsite and was hospitalized as a result of the burns. The hospitalization was not reported within 24-hours to the local Area Office of the Occupational Safety and Health Administration. The Houston South Area Office was notified on May 22, 2024, at approximately 5:30 p.m.
