29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a). On or about May 28, 2024, and at times prior, employees were exposed to slip, trip, and fall hazards while working in and around zones 1 and 2 repicks area associated with a chunk of concrete floor missing measuring 2 inches deep, 3 inches wide, and approximately 3 feet long.
