29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about May 28, 2024, the employer did not ensure that energy control procedures were developed, documented and utilized prior to allowing employees to perform service and maintenance activities on the Eastey Automatic Bundling Machine (EB35-AV5- REV F/Serial #: EB35A231001). Employees were exposed to crushing and burn injuries while clearing jams and refeeding the plastic wrap through the rollers and heat seal head. The machine was not de-energized and locked out to prevent the unexpected start-up during this servicing and maintenance activity. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about May 28, 2024, the employer did not ensure that the heat seal head on the Eastey Automatic Bunding Machine (EB35-AV5- REV F/Serial #: EB35A231001) was guarded to prevent access while the line was in operation. Employees were exposed to crushing and burn injuries when the heat seal head cycles, creating a caught between hazard between the heat seal head and the conveyor line. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
