29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: Location - Central Sterile Reprocessing and Endoscopy Sterile Processing. On or about 5/28/2024, the employer failed to provide written certification confirming that workplace hazard assessments had been conducted for the following tasks: a. Disinfecting the flexible endoscopes using hazardous chemicals. b. Changing out the containers of the Rapicide PA and other chemicals from the Medivators when empty. c. Disposal of the RAPICIDE PA Parts A and B. d. Use of the SSS Navigator #62 Perisept Sporicidal Disinfectant Cleaner.
