OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and/or amputation hazards: a) Walmart Store #2555, 117 Wilton Boulevard, New Castle, DE 19720: On or about 05/28/2024, the access gate to the vertical downstroke compactor/baler with an inoperable interlock to prevent access to the moving ram during operation, as observed on or about May 28, 2024. A gap of approximately 1 inch to 1-1/8 inches (measured from left to right) between the bale chamber door and loading chamber closure allowing am employee to access the ram during operation amputating fingers.
