(a) Worksite - Exterior Scaffold: On or about 5/29/2024, two employees were working from a scaffold platform approximately 10-feet 6-inches above the lower level that was not fully planked.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. (a) Worksite - Exterior: On or about 5/29/2024, an employee's personal fall arrest lanyard was wrapped around a horizontal scaffold rail and connected to itself. (b) Worksite - Exterior: On or about 5/29/2024, the foreman's personal fall arrest lanyard was attached to a cross-brace connection point of the scaffold system.
