29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) Omar's Carpentry & Framing Company failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:   Citation 1, Item 2 a serious violation of 29 CFR 1926.503(a)(1) with an abatement date of August 27, 2024.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about May 29, 2024, at a jobsite located at 7980 W. Preserve Parkway, Mequon, WI 53097, employees installing decking and weatherproofing on an approximately 3:12 pitch roof were not wearing fall protection. This exposed employees to a fall hazard of approximately 20 feet from roof to the ground below on the front, left, and back sides of the home.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  (a) On or about May 29, 2024, at a jobsite located at 7980 W. Preserve Parkway, Mequon, WI 53097, the employer did not ensure that all employees installing decking and weather proofing were trained to recognize fall hazards and how to implement fall protection methods, exposing employees to fall hazards.
